Fear: the foundation under all of itडर: सबैको आधारशिला

Fear is adaptive; the amygdala's alarm keeps you alive. Anxiety disorders are fear that has become untethered from real danger; the OCD spectrum is what happens when a person finds a behaviour that switches the fear off, and the relief reinforces the behaviour. Learn these two diagrams once and you can explain eight disorders with them.

Fight-or-flight. The same body responses that save you from a real threat become the "symptoms" a client brings to you, palpitations, tingling, breathlessness, when the alarm fires without danger.

The negative reinforcement loop. Phobic avoidance, compulsive washing, reassurance-seeking, mirror-checking in BDD, even bingeing after restriction: the mechanism is one and the same. Treatment (exposure) works by breaking the loop at the safety-behaviour link.
Key points
  1. Fear is the body's alarm; it is useful when danger is real and a problem when it fires without danger.
  2. Avoidance brings relief, and relief teaches the brain to avoid again: this loop keeps anxiety alive.
  3. Facing the feared thing in small steps, with support, is how the loop is broken.
