These are what a trained counselor does with a person over several sessions, laid out in order so a student can see the shape of each. Acceptance and Commitment Therapy (ACT), developed by Steven Hayes, starts from one idea: the problem is rarely the anxious feeling itself, it is the struggle to get rid of it. The struggle shrinks a life. ACT works through six processes, usually woven together rather than done in order:

  • Values: what kind of person, parent, worker, friend the client wants to be.
  • Committed action: small steps in that direction, taken with the anxiety still present.

One worked illustration (fictional): a young woman who skips meetings at work because she is afraid of being judged. The class notes teach it as nine steps; a real course of sessions loops back and forth between them.

  1. Rapport and the pattern. "I want to understand what you are going through. There is no judgement here." Ask what she does when the nervousness comes. The answer, "I skip the meeting", shows the loop: anxiety, avoid, short relief, more anxiety next time.
  2. Explain it simply. "Anxiety is normal. The problem is not the anxiety, it is running from it." The picture: quicksand, the harder you struggle, the deeper you sink.
  3. Acceptance. "Instead of fighting the anxiety, let us learn to make space for it." Where is it in the body? "In my chest." "Let it be there. You do not have to remove it." (Eyes open is fine; closed eyes are optional.)
  4. Defusion. "Thoughts are not facts. They are words in your mind." The thought "I will embarrass myself" becomes "I am having the thought that I will embarrass myself." Said aloud many times, the sentence loses its grip.
  5. Present moment. "Let us focus on what is happening right now." The 5-4-3-2-1 grounding from page 23, used in the minutes before a meeting.
  6. Values. "What kind of person do you want to be?" "I want to grow in my work." "Is skipping meetings helping you grow?"
  7. Committed action. "You can feel anxious and still act." A ladder of small steps: say one sentence, ask one question, share one idea.
  8. Practice between sessions. "Practice matters. Small steps make the change." Attend one meeting; say the defusion sentence once a day.
  9. Review. "It is all right if the anxiety is still there. What matters is what you do." The expected outcome is honest: the anxiety may remain, the behaviour changes, confidence follows.

Where it is used: anxiety disorders and depression have the best evidence. For OCD, ERP remains the first-line treatment and ACT is an addition to it; for trauma and substance use it is an adjunct, with a trained provider, not a stand-alone answer.

Depression makes a person do less; doing less makes the mood worse; the worse mood makes them do even less. That is the spiral on page 08. Behavioural activation breaks it at the easiest link to move: the activity. The one line to remember is action before motivation: the client acts first, and the wish to act returns later. The class notes teach nine steps.

  1. Understand the day. Sleep, meals, who they talk to, what they have stopped doing.
  2. Explain the cycle. "When we do less we feel worse, and when we feel worse we do less. We break it with action."
  3. Monitor activity. For a few days the client writes what they did each hour and how they felt.
  4. Schedule activity. A daily plan of small, realistic tasks. Not perfection, movement.
  5. Activate. Do the planned thing even when it does not feel like it. This step is the therapy.
  6. Add pleasure and achievement. One thing that is enjoyable, one that gives a sense of having done something.
  7. Solve the barriers. Tiredness, fear, no support: each named and worked around.
  8. Practise between sessions. This is where the change actually happens.
  9. Review. Every small gain counted out loud.

The chart is the tool. How to introduce it: "This chart helps us see how what you do affects how you feel. Nobody is judging you; we are learning what works." Activities fall into four kinds: routine (bathing, eating, sleeping), pleasure (music, a film), achievement (cleaning, finishing a task), social (talking to family or friends).

Read the first chart with the client, not to them: the walk scored higher than the phone on all three columns, and they wrote that themselves. The "No" on Tuesday is information, not failure; ask what got in the way.

Exposure and response prevention (ERP) is the behavioural treatment for OCD. It rests on the loop drawn on page 07: trigger, obsession, anxiety, compulsion, short relief, and the loop tightened for next time. ERP breaks it at the compulsion. This is a treatment delivered by a trained person over weeks, with consent at every step; what follows is the shape of it, not a kit to run alone; the ladder below is one example a trained clinician built with one person after assessing real risk, not a template.

First, three things the client hears: an intrusive thought is frightening, but it is not an action and not a fact; the ritual calms for a minute and feeds a problem that can cost skin, sleep, work and mood for years.

The session. Start on the lowest rung. The client touches the table and stays with the feeling: "Stay with the anxiety. Do not wash your hands yet." "Notice the thoughts without acting on them." The aim is not to make the anxiety fall; it is to stay long enough to learn that the feared outcome is less likely, or more bearable, than the fear predicted, and that the anxiety, whether it falls or not, is survivable. Slow breathing may steady the body, but it is not used as an escape hatch from the feeling. SUDS is noted before, during and after, so the client can see their own record over the weeks.

Response prevention is the principle that makes it ERP rather than just exposure: after the trigger, no washing, no sanitiser, no avoiding, or, where the clinician judges it right, an agreed delay that is then shortened. Practice is repeated and varied; the ladder moves up when the person has learned something new about the fear and feels ready, not when boredom sets in.

For the counselor: never rush; always start low; consent and readiness before every rung; support the person warmly but do not reassure them about the fear itself ("you will be fine" is a ritual by proxy); keep the record. Alongside, gently test the thought: "If I do not wash I will get sick" becomes "Ordinary washing, before meals and after the toilet, continues; the extra washing is the OCD talking, and I can carry the not-knowing." Expect a stray intrusive thought to return now and then after treatment; the plan is not to go back to the ritual, and to keep the practice going.

The class notes teach "life skills counselling" as a ten-step course; almost every step is already taught in full elsewhere on this site. The counselor's job is to find which skill is weak ("Which situations do you feel unable to handle?"), say plainly that skills are learned, not inborn, then teach, rehearse by role-play, set practice, and give feedback. Six areas:

Five steps through a strong feeling. Notice early: feelings climb like a traffic light, green, yellow, red; catch it at yellow, the way you catch milk before it boils over. Your body gives the signal first: faster heart, quicker breath, a tight jaw. Find it: eyes open is fine; where does it sit, shoulders, chest, jaw? Let that place soften a little. Name it: anger is often a shield; ask "is this anger, or is there hurt, fear or shame underneath?" A named feeling loses some of its force. Breathe out long: in through the nose, out slowly and a little longer than in; no holding, no counting contest, stop if it feels wrong. Ask what it needs: turn it into an I-message: "I feel ___ because I needed ___; next time, could you ___?" Use this only where it is safe to speak plainly; where there is violence or control at home, safety comes first (page 27). Later, if it helps, write down what happened, what you felt and what you needed, facts first, blame left out.

  1. ACT does not remove the anxiety; it loosens the struggle with it and moves the person toward what they value, one small committed step at a time.
  2. Behavioural activation runs on one rule, action before motivation: the activity chart shows the client, in their own handwriting, that what they do moves their mood.
  3. ERP faces the trigger and skips the ritual; the point of staying is to learn that the feared outcome is less likely or more bearable than predicted, and that the not-knowing can be carried; not to force the anxiety down. Never reassure about the fear itself.
